I need some help deciphering Anniversary Mix Growth Chart?

So I have a question about the growth chart, I keep on getting the snail teen, could someone who knows Japanese assist me and tell me what the writing means?

I'm guessing the teen on the far left is perfect care and this chart ranges from perfect care on the left to horrible care on the right. I can't read it either, but I'm assuming the snail teen is the worst care teen.

But what does the 20 bit mean? I can guess the 4-5 etc... But that's one thing I don't get
