"There are three kinds of bad care.
Over-feeding:
1. Feed meals until the characters refuse to eat any more.
Note: There are several hidden hungry hearts.
2. Wait until the characters are willing to eat again, then repeat step 1.
Note: It may take about an hour before they are hungry again enough to eat.
3. Repeat steps 1 & 2, at least 5 times. When you've repeated this enough, they will change into bad care characters as soon as they've finished eating a meal.
Under-feeding:
1. Allow all hungry hearts to empty.
Note: Keep some happy hearts full at all times.
2. When they beep for attention because they're hungry, don't feed them.
3. After some time, they will appear angry with you, but don't feed them.
Note: It may take 15-30 minutes from the time they call for attention until they appear angry with you.
4. After some more time, they will appear no longer angry with you. Now, feed them a meal that will fill one hungry heart.
Note: It may take 15-30 minute from the time they first appear angry until they no longer appear angry.
Note: The attention icon may remain lit after they no longer appear angry with you, before you've fed them anything.
Unhappy:

I believe bad-care during pre-adult stages does count. However, they won't change into bad-care characters until after they become adults. I don't recall if they become adults then immediately change into bad-care characters, of if it requires one more bad-care to trigger the change."
